A Clinical Analysis on Periampullary Cancer


Abstract
Since periampullary cancer often presents late clinical manifestation and diffiult anatomical location, the correct preoperative diagnosis is dificult to make. Despite recent addvance in medical knowedge and surgical techniques, the prognosis is
poor.
Atuhors evaluated 68 patients who received operation for periampullary cancer in the Department of Surgery at Pusan National University Hospital, from January 1986 to December 1991.
@ES The results were as followings:
@EN 1) Among 68 cases, 38 cases (55.9%) were pancreatic head cancer, 18 cases (26.5%) were distal CBD cancer, 12 cases (17.6%) were Ampulla of Vater cancer.
2) The male and female ratio was 1.5:1. The prevalent age was 6th decade (32.4%), and mean age was 51.9-year-old.
3) The major clinical symptoms and signs were abdominal pain (66.2%), anorexia and indigestion (64.7%), jaundice (52.9%), weight loss (47.1%), fever and chilling (33.8%)
4) Improtant laboratory findings were increased alkaline phosphatase (73.5%) bilirubin (66.2%), AST and ALT (61.7%).
5) The valuabe diagnostic methods were CT, PTC, ERCP and Ul trasonography.
6) Adenocarcinoma was the most common type (97.1%).
7) Pancreaticoduodenectomy was performed in 38 cases (55.9%), total pancreastectomy in 2 cases (2.9%), bypass procedures in 22 cases (32.4%) and biopsy only in 6 cases (8.8%).
8) Postoperative complications were wound infection (13.2%), pulmonary complications, pancreastic and bile leakage, bleeding etc.
9) The operative mortality was 12.5% in curative operation and 10.7% in palliative operation.
10) The mean survival was 8.1 months in pancreatic head cancer, 23 months in Ampulla of Vater cancer and 22 months in distal CBD cancer.
